Closing Service Gaps: Survey on Homelessness Prevention Services and Community Resources

The following survey was developed by The New Jersey Child Placement Advisory Council (CPAC) Partners Moving Forward Committee.  The objective of the survey is to examine how we may better coordinate efforts to prevent and address homelessness, particularly of families, children and youth, in our state.

For our purposes, the term “homeless” refers to parents without permanent housing; children with families who are without permanent housing; and unsheltered youth unaccompanied by a family.

The survey is anonymous. Please respond to the questions as your organization provided services prior to the COVID--19 pandemic. Please complete each question before proceeding to the next one. Thank you for your participation.
