Hi!  You may not remember all of these activities as each section of Grade 8 Health did not study exactly the same topics in exactly the same order.  Please fill in the questions as clearly as you can.  There will be space at the end of the survey to add any additional comments to further explain any of your answers that you feel needed more explanation.  This survey does not affect your mark.  You can be honest.  That does not mean you can express yourself rudely.  Hopefully you have learned how to communicate in a healthy way and will apply that to this survey and all aspects of your life. I (Ms Maxwell) am conducting this survey so that I can get a "report card" on the Grade 8 Health course.  This will help me know what parts I should keep for next year and which parts I may want to alter or replace with something else more important (that still fits within what the government documents tell Health teachers they MUST teach!)  Even though I have been teaching for 19 years, I still like to hear new perspectives on how students fell they benefit or do not benefit from various course activities!  Your participation means that I can self-improve (and that is a healthy thing to want to do)!  Thanks/merci!
