Candace L. Carmichael Award for the Conference on Aging
The Cuyahoga County Conference on Aging Committee requests nominations for the Candace L. Carmichael Award (for individuals) and/or the Barbara S. Galloway Award (for organizations). The awards will be presented at the Conference on Aging on Friday, October 9, 2015.
The Candace Carmichael award recognizes an individual who exhibits meritorious and innovative service to seniors in Cuyahoga County and honors the legacy of a leader and program developer for seniors in the field of aging.
The Barbara Galloway Award recognizes an organization that has made significnat contributions to the aging network and honors the legacy of a community leader and pioneer in the field of aging.
Guidelines:
1. The nominee must be a resident of, or organization doing business in, Cuyahoga County, Ohio for at least (5)
five years.
2. Emphasis will be placed on contributions that have improved the quality of life for seniors and adults with disabilities
in Cuyahoga County.
3. Innovative and meritorious service in community programs and/or individual activities which continue to have a
significant impact on behalf of seniors and adults with disabilities will be considered.
4. Provide a description of why you feel this individual or organization should receive the award.

Barbara S. Galloway Award - Community Partnership on Aging (South Euclid, Lyndhurst, Mayfield Hts., Mayfield Village, Highland Hts.)
Barbara S. Galloway Award - Community Partnership on Aging (South Euclid, Lyndhurst, Mayfield Hts., Mayfield Village, Highland Hts.)
