This survey is part of the Self-Study Report for Initial and Continuing Accreditation assessment. The CoA-SBBT/TM requests completion by each key faculty member (Program Director, Medical Director, & Education Coordinator), at least one didactic and one clinical instructor, and other program representatives at the discretion of the Program Director.
