Purpose: We want to make Natick a healthier place for you and your family. Please take this survey to tell us about your health needs and your life in Natick. Your answers will help Natick is United create better services, events, and programs for all immigrant neighbors. Thank you for helping our community.
Confidentiality: This survey is anonymous. We will not ask for your name or any information that could identify you. Your answers will be combined with responses from other community members and reported only as group data. Participation is voluntary, and you may skip any question.
We ask that you answer these questions based on your own personal experiences. The only exception is the question that asks if a family member has experienced discrimination.
Estimated Time: Approximately 15-20 minutes.
Language:This survey will be available in English, Spanish, Portuguese, Mandarin/Chinese, and other languages based on community need.
