Survey about Maternity Care in NWT
The Government of the Northwest Territories Department of Health and Social Services has contracted DPRA Canada to conduct midwifery sharing circles in the NWT.
You are being asked to complete a survey about:

1. Your experiences with maternity care in NWT; and
2. Your thoughts about midwifery in NWT.
The survey will take about 10-15 minutes to complete.
Your responses to the questions are completely anonymous. We will not ask your name or contact information at any point. If you do not want to answer a question, you can skip it. The answers you give will be stored in a secure, password protected database.
