Survey for J-1 Program Supervisor

To comply with J-1 visa regulations, the program supervisor is REQUIRED to complete a final evaluation for all J-1 programs, as well as a midpoint evaluation for six months or longer in length. The survey should not take more than 5 minutes and should be completed prior to the end date of the trainee/intern’s program as noted on their Form DS-2019. Please make sure to answer the questions completely.
