Greetings! The Healthy Union Advisory Coalition is gathering input from leaders who work in the area of mental health in Union County. We would appreciate your feedback on this brief questionnaire prior to December 10th 2021.

Your responses will inform the Union County Community Health Improvement Plan (CHIP). Your personal contact information will remain confidential.
 
Our group's work stems from the tri-annual Community Health Assessment (CHA) and our main goals are to listen to what is happening in our community, identify available resources to improve access, recognize gaps in services, and bring back insights and suggestions to place in our CHIP.
