MAX Ottawa Mental Health Needs Assesment

We invite you to take part in this survey as someone who has participated in the Tea2Go program offered by MAX Ottawa.
This survey is part of an evaluation of the Tea2Go program. Its purpose is to better understand clients’ needs and gather insights that will help MAX Ottawa strengthen and improve the program.
You will not be asked to share your name or any identifying information. Your responses will remain anonymous.
Participation is completely voluntary, and you may skip any question you prefer not to answer. Completing this survey is
not
required to access Tea2Go harm‑reduction services, and taking part will
not
affect your ability to receive services.
The information gathered will be used by MAX Ottawa and the evaluator, Selena Houmani, for the purpose of evaluating and improving the Tea2Go program. This survey was developed for the purpose of this evaluation, and its content will be used to support the needs assessment. When results are shared, your answers will be grouped with those of other participants, and no names or personal identifiers will appear in any reports.
Data from this survey will be stored securely by MAX Ottawa for five years after the evaluation is completed. Only the Tea2Go team and the evaluator, Selena Houmani, will have access to this information.
