1. Introduction

Dear Health Center,

The purpose of this 2016 Needs Assessment is to identify performance requirements or needs with health centers servicing public housing residents, including HRSA Health Center Program Grantees. We seek this information so that we may direct North American Management’s resources to the areas of greatest need, help organizations increase revenue, reduce cost, improve productivity and provide optimal patient care to improve overall patient health outcomes. Lastly, we want to help ensure that HRSA Health Center Program Grantees comply with HRSA’s required Program Expectations.

Your individual responses will be kept confidential, will not be shared with other agencies, and will not impact any funding decisions.  We will compile all responses and only aggregate non-identifiable data will be shared with other BPHC grantees and HRSA. This needs assessment will take 30-60 minutes to complete.

As a reminder, the National Center for Health in Public Housing provides training and technical assistance on HRSA 19-program requirements and emerging issues affecting Health Center Programs providing health services to public housing residents.  You will be contacted by a staff member to facilitate your needs for training and technical assistance if requested.
