Q&A Forum Moderator Evaluation Form

Each time you use a Q&A Forum Moderator, we ask that you complete an evaluation of the Moderator’s performance. This evaluation will be held in strict confidence and will be used by NACVA as a tool to monitor and improve the quality of our Q&A Forum services. Please take a few minutes to answer the following questions:

Prompt (less than 2 hours)

Reasonable (the same day)
