Invitation to Participate: You are invited to participate in a study of Early College participation
being conducted by Danielle Gibson, Faye Kilday, and Kayla Burdick through Northeast Community
College. Your participation is completely voluntary.
Overall Purpose of Study: Northeast Community College’s goal is to increase the college-going
rate in our region by increasing access to Early College for underserved students earlier in their
educational journey. We will use this research to begin to understand possible barriers for these
students and then make improvements to current Early College programming.
Basis for Participant Selection: You have been selected for participation in this study because
your student’s high school is a committed partner, and your student is not a current Early College
student at Northeast Community College.
Explanation of Procedures: If you decide to participate, you will be asked to do the following
things: Answer an informed consent question, opt-in to survey, answer qualifying question,
participate in survey (if qualified), provide consent at completion of survey for child by providing
student contact information, and be provided the option to print/save a copy of your survey.
Potential Benefits: Potential Benefits: Survey responses will assist Northeast Early College in better understanding and serving current and future students while providing a model for other post-secondary partners and organizations.
Potential Risks and Discomforts: The risk of this survey will be minimal to the participants due to
the confidential nature of the invitation and delivery of the survey via SurveyMonkey.

Assurance of Confidentiality: To protect your rights, your information will be kept confidential and
will not be released to any person or agency without your expressed permission unless required by
law. Any shared findings will be presented in a way that will not identify you. Data will be accessed
from survey software data cloud through an integration with Power BI and stored in a Survey
Software Data Cloud for a minimum of 5 years, then disposed of according to Northeast
Community College’s Data Retention Policy.
Although every reasonable effort will be made, confidentiality during actual Internet
communication procedures cannot be guaranteed.
Future Research: It is possible that identifiers may be removed from the identifiable private
information and that, after such removal, the information could be used for future research studies
or distributed to another investigator for future research studies without additional informed
consent from the subject or legally authorized representative.
Withdrawal from the Study: Your participation in this study is voluntary. Your decision whether or
not to participate will not affect your current or future relationship with Northeast Community
College, Danielle Gibson, Faye Kilday, or Kayla Burdick. If you decide to participate, you are free to
withdraw your consent and discontinue participation at any time.
Offer to Answer Questions: You should feel free to ask questions now and at any time during the
