SERVICE PROJECT OPTIONS

DUE AUGUST 29TH AT NOON

Please use this form to submit your two (2) preferred service projects for Day of Caring.

Before beginning the Preference Form, see below:
To ensure that Day of Caring is an event supporting the needs of our non-profit partners by providing the help and resources that they NEED, the following information is important:
  • We will first consider the capacity of our agency partners when assigning volunteer groups.
    • We will not send groups larger or smaller than the agencies have requested.
  • We will do our BEST to assign your group to one of your 2 preferences.
    • There is no guarantee.
    • We will assign things as needed to ensure all agencies are covered.
  • It is possible that your group will have to split into more than one job site:
    • Especially if you have a large group
    • If you do not submit this form on time - we will begin the process of assigning projects at NOON on August 29th. If e do not have this form at that time then we will have to assign your Company/Group to the remaining projects once we receive your form. Matching groups to projects takes a lot of time and we will need to stay on task to complete it in a timely manner. Submitting your preferences by August 29th at NOON is critical.
  • Please provide all of the information requested below
  • If you have any questions please feel free to email cdaviess@uwheartmo.org
Before you click the link below, remember that there are two (2) tabs at the bottom of the spreadsheet labeled "Project Options" and "Supplies Drive" click on each tab to view each list:

Question Title

* 3. GENERAL COMMENTS - PLEASE USE THIS SPACE TO LET US KNOW ANYTHING THAT IS IMPORTANT AS WE ASSIGN PROJECTS:
EXAMPLE:
We do not need to stay together at the same job site.
EXAMPLE:
We do not mind having an A.M. job site and moving to a different job site after lunch.
EXAMPLE:
If our job finishes early we would like to be directed to another job site that could use our help.
EXAMPLE:
Our group has 8 registered volunteers.
  • 4 a.m. volunteers from 9-1 (they will eat lunch at the site before leaving at noon)
  • 4 p.m. volunteers from 1-4 (they will arrive at noon to eat lunch with the a.m. volunteers)

Please remember to keep in mind the NUMBER OF VOLUNTEERS AN AGENCY HAS REQUESTED BE ASSIGNED TO THEIR PROJECT. All volunteer numbers on projects are for the duration of the project. So 10 volunteers means that they need 10 all day, not 5 in the a.m. and 5 in the p.m. If your group can only provide 5 of the 10 requested volunteers then your group may be working with another group at that project site.

* 8. WILL YOUR COMPANY OR GROUP BE ADOPTING AN AGENCY AND COLLECTING NEEDED SUPPLIES FOR THEM?
(Please keep in mind that if you'd like to adopt an agency but feel that September 11th doesn't give you enough time to collect their needed items, you can request a different date. We will work with the agencies to arrange that for your convenience.)
