Thank you for registering your interest to support the Department of Environment, Energy and Climate Action (DEECA) in the event of an Emergency Animal Disease outbreak. In order to understand which of our two engagement pathways are best suited to your needs please complete this form.

Service Level Agreement - Recommended for sole traders and veterinary practices with professional indemnity and public liability insurance (to the value of $10 million). You can view the Service Level Agreement here. 

Casual Employment Contract - Recommended for individuals who do not have professional indemnity and public liability insurance (to the value of $10 million).

DEECA is collecting your personal information such as your name, contact details and business information to assist with resourcing as part of preparedness for an Emergency Animal Disease incident.  Your personal information will be stored on secure databases which reside in Australia and the USA. Any information provided will be used in accordance with the Privacy and Data Protection Act 2014 (Vic) (PDP Act), Public Records Act 1973 (Vic), Health Records Act 2001 (Vic) and other applicable legislation.
