The Thunder Bay District Health Unit and Thunder Bay Regional Health Sciences Centre (including the Maternity Centre) are completing an environmental scan of local health care providers and community organizations to gain an understanding of what resources, programming and services are currently offered within the District of Thunder Bay as well as current gaps to prenatal programming that may exist. This will help us to revise our current prenatal programming. This survey is for health care providers, pharmacies and community organizations within the District of Thunder Bay who provide resources, programming or services to prenatal clients (clients who are pregnant or the support partner to a pregnant person).
 
This survey consists of about 15 questions that will only take about 5 minutes to complete. The survey will be open until Tuesday, June 21, 2022 at 11:59 pm. Your participation in this survey is voluntary. All information will be held in the strictest confidence. No personal names or identifying information will be used in any analysis, publication or presentation.
