Participant Information and Consent Form
 
Name of Project: Family perspectives of post school experiences of young people with disability in Australia.
 
You are invited to participate in a study of family perspectives of the post school experiences of young people, aged 18 to 30 years, with disability in Australia.  The purpose of the study is to gain an understanding of what parents/guardians think about the post school experiences of their young adult children with disabilities. We want to know how these compare to their experiences in the early childhood and school years. We will use the information we collect to identify how post school services for people with disability could be improved.
 
The study is being conducted by Dr Sarah Carlon (Ph: 0411 640 408; sarah.carlon@mq.edu.au) and Dr  Coral Kemp (Ph: 02 9850 8712; coral.kemp@mq.edu.au) of the Macquarie University School of Education and the STaR Association, with Mrs Margaret Meaker (Ph: 02 8850 1269; margaret@star.org.au) and Dr Roshini Chanderoo (Ph: 02 8850 1269; roshinichandroo@gmail.com) of the STaR Association.
 
If you decide to participate, you will be asked to complete an anonymous online survey. It will take around 10 - 15 minutes to complete.
 
Participants in the study will be given the opportunity to provide contact details to go into a draw to win one of three $100 Coles Group/Myer gift vouchers. Participation in this study and the provision of contact details for the draw are both entirely voluntary. Please note that completion of the survey indicates your consent for data collected through the survey to be included in this research project and resulting publications. When you have read and understood the requirements of this survey and are happy to proceed, please click NEXT.
 
You will also be provided with the opportunity to indicate whether you are interested in participating in a follow-up study involving interviews with parents of young adults with disability. Indicating your interest does not commit you to participating in the follow-up study. The researchers will simply contact you at a later date by phone or email to invite you to participate. If you choose to provide your contact details for the prize draw or to indicate your interest in the follow-up study you may click on the link at the end of the survey to provide your details in a separate online form- your identity will not be linked to the survey you have completed.
 
Any information or personal details gathered in the course of the study are confidential, except as required by law.  No individual will be identified in any publication of the results.  Only the research team will have access to the data. A summary of the results of the data can be made available to you on request and will be published on the STaR website.
 
Participation in this study is entirely voluntary and, as you will not be identified in the survey, information provided by you cannot be shared with your current post school service provider.
 
The ethical aspects of this study have been approved by the Macquarie University Human Research Ethics Committee (Reference No: 52020677119162; Project ID 6771).  If you have any complaints or reservations about any ethical aspect of your participation in this research, you may contact the Committee through the Director, Research Ethics & Integrity (telephone (02) 9850 7854; email ethics@mq.edu.au).  Any complaint you make will be treated in confidence and investigated, and you will be informed of the outcome.
