Welcome to the Portland Public Schools Nellie Mae Education Foundation student questionnaire. This survey is part of Education Development Center's evaluation of Portland's implementation of student-centered learning. For the purposes of this questionnaire, the phrase “Student-centered Learning” refers to your experiences with learning that happens Anytime/Anywhere, that is Personalized to meet student needs and interests, that is based on demonstration of Proficiency in what you're learning, and that supports an increased role for Student Voice and Leadership in what and how you learn. Some or all of these features of student-centered learning may be part of your school's effort to insure that your learning prepares you for college and career. Your responses will help us gather information about the types of instruction and student learning that you experience at your high school. The questions will ask you about your classroom experiences, your use of learning technology, and what it’s like to be a student at your school.

Participation in the questionnaire is voluntary, all responses are confidential, and all answers from the questionnaire will be reported collectively with answers from other students who participate in the study.
