AGREEMENT TO PARTICIPATE: In a survey to assess pilot training, aircraft understanding, manual flight, aviation passion, and safety culture.

RESEARCHER AND COMMITTEE: Dissertation research project led by Karlene Petitt, doctoral student at Embry-Riddle Aeronautical University—Worldwide. Committee: Dr. David Esser, Dr. Haydee Cuevas, Dr. Alan Stolzer, and Dr. Tony Kern.

PURPOSE: The purpose of this study is to identify the relationships between safety culture, pilot training, aircraft understanding, aviation passion, and the impact on automation usage, in order to identify the root cause of performance issues, beyond pilot error.

ELIGIBILITY: You must be a commercial pilot (airline, charter, corporate), operating an aircraft with a required crew complement of at least two pilots, and be older than 18 years of age. You may also be retired or between jobs if you were actively employed within the previous calendar year.

PARTICIPATION: You will be asked to complete a survey to assess your aviation passion, employment, training methodologies, automation usage, safety culture, and aircraft understanding, along with questions concerning demographic data. The completion of the survey will take approximately 10-15 minutes.

VOLUNTARY PARTICIPATION: Your participation is voluntary, and you can withdraw from the study at any time, or not answer any questions that you do not feel comfortable answering.  Your decision whether or not to participate will have no effect on your current or future connection with anyone at Embry-Riddle Aeronautical University. If you opt out after beginning the on-line survey that data will be removed as if the survey was never started. If you opt out on the paper survey, that paper survey will be collected and destroyed as if it never occurred.

RISKS OF PARTICIPATION: Risks of participating in this survey will be no more than every day life.

BENEFITS OF PARTICIPATION:  The benefit to participants will be the positive impact that this research could have on your career and safety of our aviation industry.

RESPONDENT PRIVACY: You will remain anonymous. Your name will not be collected on the survey, ensuring your identity will never be linked to the results. Your employer will not be identified, and the data gathered will not connect responses to a specific employer. The online survey system will not save IP addresses. For paper surveys, your completed survey will be placed into an envelope with other surveys, ensuring that there is no identity connected to each survey. After analysis all paper surveys will be destroyed.
