Introduction

CNETS Canada's National Patient Input Consultation: Everolimus (Afinitor) in Gastrointestinal (GI) or Lung Neuroendocrine Tumours (NETs).

This survey is for patients who have Gastrointestinal or Lung Neuroendocrine (NET) cancer or have experience with the cancer drug (Everolimus) Afinitor.

CNETS Canada will use the survey results to develop its submission report to the pan-Canadian Oncology Drug Review (pCODR).

This anonymous survey is your opportunity to raise issues important to you.  CNETS Canada will include your issues in CNETS Canada's patient group input submission to the expert body that reviews prescription medications and recommends whether governments' public drug insurance plans should cover the costs of prescription medications.  CNETS Canada will review and analyze your survey responses, conduct further research, draft a submission, and then send it to the Canadian Agency of Drugs and Technologies in Health (CADTH). CNETS Canada's submisison--including your input--will be considered by CADTH’s independent advisory body, the pCODR Expert Review Committee (pERC), during their deliberations when making public drug insurance plan listing recommendations. 

The demographic questions at the beginning of the survey (e.g., gender, age) will be used to provide real-world context to your experiences and opinions. Rest assured that your responses will remain anonymous. We do not collect your IP address and do not require your name.  Many of the questions include a text box so that you can provide details about your experiences and opinions. We have included these text boxes because CADTH specifically asks patient groups to provide quotes in our submission. Where we use quotes in the CNETS Canada patient submission, we will use the following format, for example: “-- 40 year old man, currently using Everolimus for GI NETs".

Please note: By submitting your feedback to these questions, you grant CNETS Canada the right to quote the text you provide in the CNETS Canada's submission and to anonymously attribute the text as demonstrated in the example above.
