1. Measures of Processes of Care-20 (MPOC-20) CanChild

We would like to understand and measure your experiences and perceptions of the care you and your child have been receiving from Pathways Health Centre for Children, at Pathways or from our Pathways staff in schools, community sites, daycares and hubs. It should only take about 10 minutes or less.
There is no information on the survey that identifies you or your child, so the answers you provide are confidential.
MPOC-20 CanChild: The Measure of Processes of Care is a well-validated and reliable self-report measure of parents' perceptions of the extent to which the health services they and their child(ren) receive are family-centred. MPOC has been used internationally in many evaluations of family-centred service.
"PEOPLE" refers to those individuals from Pathways who work directly with you or your child.

The "CENTRE" refers to all staff from Pathways Health Centre for Children, whether involved directly with your child or not. In addition to health care people, they may include support staff such as office staff, custodians, administrative personnel, etc.

For each question, please think about how much the event or situation happens to you. You are asked to respond by selecting from one of the choices (To a Very Great Extent down to Not at All) that you feel best fits your experience. Please select Not Applicable if the situation described does not apply to you.
