Welcome to our survey for women with ovarian cancer

Thank you for agreeing to take part in our Pathfinder Study. This is one of five surveys making up Pathfinder 2016. The other four surveys are for:

·        The general public
·        GPs
·        Clinical Nurse Specialists
·        Family and friends

These five surveys will provide a UK wide picture of awareness of ovarian cancer, diagnosis, treatment and palliative care. They will help us ensure that our services here at Target Ovarian Cancer best meet the needs and ambitions of women with ovarian cancer while also providing the basis for our campaigning work. For the first time ever we also plan on publishing separate reports for England, Scotland, Wales and Northern Ireland.

This survey is for women living in the UK who were first diagnosed with ovarian cancer either during or since 2010. This is to make sure the information we collect is as recent as possible. If you were diagnosed before 2010 you are still welcome to complete the survey and we will use your response to help inform our work, but it will not be part of the Pathfinder 2016 reports.

In order to find out as much information as possible the survey is quite long. You may also find some of the questions hard to answer or they may bring back difficult memories. We therefore suggest finding a quiet time and place to complete the survey and please feel free to leave any questions you would rather not answer. Our contact details are at the end of the survey and please do get in touch if you would like to talk to anyone after completing it.

While there is a chance to provide your contact details at the end of this survey so you can stay up to date on our work, all published data will be anonymised.
