Park Grove Library Project Questionnaire

Thank you for your interest in this questionnaire. Toward the end of the questionnaire, you'll notice OPTIONAL demographic specific questions. By completing these questions, you are helping Washington County to comply with Title VI and related statutes, which require the collection of statistical data to aid in assessing outreach efforts among those who are affected or interested in this project.
