Please rate your level of satisfaction regarding the following topic areas as they relate to Melmark New England’s servicing of you and your child. Some statements will require you to rate your level of satisfaction based on your general impressions, your observations at the school, your interaction with staff and/or the progress of your son or daughter. Statements have been grouped into the following topic areas:

Two distinct and yet similar surveys have been developed for students who attend only the school setting and those attending residential services. Your comments, as well as your rating, are most appreciated. We are viewing this satisfaction survey as a means of assessing the areas where program development is strong and where it needs improvement. Your accurate assessment, including constructive comments, is necessary to promote quality program development.
There may be some statements that are not applicable to you and your child and therefore please make this notification. After the survey has closed, we will summarize the information from all parents and forward that summary to you. We will also personally address any comments and issues for individuals who have identified themselves.
