This survey is designed to collect information crucial to efforts to reduce unnecessary paperwork in the Child and Adult Care Food Program (CACFP). This survey covers potential areas of opportunity for paperwork reduction including applications, oversight and monitoring, recordkeeping and reporting, and technology. The information you provide for this survey will be included in a report requested by Congress.

A consortium of CACFP and child care organizations including the National CACFP Forum, CACFP Sponsors Association, Child Care Food Program Round Table, CACFP National Professional Association, Minute Menu, and the National Head Start Association have come together to collect information from a broad range of organizations sponsoring child care centers, afterschool programs, and family child care homes.
