COVID-19 Readiness Study Survey (English & Spanish)

Propósito: El propósito de esta encuesta es comprender sus necesidades como residente de la comunidad afectada por la actual pandemia de coronavirus (COVID19) en la ciudad de Nueva York. Su aporte en esta encuesta es muy valioso para los profesionales de la salud que trabajan para reducir la infección en la pandemia. Ya sea que participe o no, no afectará ningún servicio que reciba en la comunidad o su estado migratorio. Este estudio es totalmente anónimo. Las preguntas de la encuesta también tienen como objetivo informarle sobre cómo estar preparado para una pandemia. Esto solo puede tomar unos 5 minutos de su tiempo. 

Purpose: The purpose of this survey is to understand your needs as a resident of community affected by the current coronavirus (COVID19) pandemic in New York City. Your input on this survey is very valuable for health professionals working to reduce infection on the pandemic. Whether or not you participate, it will not affect any services you receive in the community or your immigration status. This study is totally anonymous. The questions in the survey is also intended to make you aware on how to be ready for a pandemic. This may only take about 5 minutes of your time.
