7.If you know someone that meets the criteria to receive a Pfizer Booster shot and would like to get them an appointment at the clinic you are working, please list their full name, email and or cell number below.
In order to qualify for the Pfizer booster, you will need to have previously received a Pfizer first and second dose vaccine. The second dose must have been received in January, February or March of 2021. You also must belong in one of the 4 categories:

· People who are 65 and older

· Long-Term Care Facility Residents

· People 18-49 or 50-64 with underlying conditions

· People 18-64 years who are at increased risk for COVID-19 exposure and transmission because of occupational or institutional setting.
