  1. Nominations must be completed and submitted by Friday August 8th, 2025 @ midnight to be considered for this year's awards.
  2. You are required to provide a write up on why you are nominating this individual or organization, including details supporting your why.
  3. You must also include the name of 1 supporting reference.
  4. The selection committee will review all submissions for approval.
Cornerstone Award
The Cornerstone Award recognizes a member funeral home that has made an outstanding contribution to the betterment of the funeral profession at large.
Excellence in Funeral Service Award
For a funeral professional member or team that has provided outstanding customer service to families.
Community Impact Award
For a member or organization who has made significant contributions to their local community through their professional work.
Rising Star Award
For young professionals who are members and have shown exceptional promise and contributions to the industry and who has made significant contributions within their first 1-2 years in the profession.
Lifetime Achievement Award
Honours a member who has dedicated their career to advancing the industry and who has provided exceptional guidance and support to other professionals.
Association Activist Award
The Association Activist Award recognizes outstanding contributions to OFSA by association volunteers.
