* 1. This survey is for parents or caregivers of children with special needs who are between the ages of three and twenty-one and who are receiving school-based services through an Individualized Education Program (IEP). The intent of this survey is to collect the perspectives and suggestions of parents of children with special needs in order to improve school relationships.  The results will be published and shared with school-based professionals (e.g., educators, related service providers, administrators, etc.) to provide information, insights and experiences of parents of children with IEPs and strategies for how to enhance relationships. 

This survey is anonymous. By completing this anonymous survey, you are consenting that the results can be shared publicly and understand that there is no way to connect your responses to your identity. The survey will take approximately 20 minutes to complete.  Completion of this survey is entirely voluntary and there is no cost or compensation for completing it. You may skip any questions that you do not feel comfortable answering and may stop participating at any time that you desire.  This survey will close on 12/31/20 and no additional survey responses will be collected after that time period.
