CONSENT FORM
You are invited to take part in a research study about the experiences that could help explain factors related to nursing retention. For this study, you are invited to describe your experiences of compassion fatigue and turnover intention through mindfulness. This form is part of a process called “informed consent” to allow you to understand this study before deciding whether to take part.
This study seeks 395 volunteers who are:
· Registered nurses living and working in the United States
· 18 years old or older
· Working at a physical location
· Employed as a registered nurse for a minimum of two years
This study is being conducted by a researcher named Heather Kenworthy who is a Doctoral student at Walden University, but this study is separate from that role.
Study Purpose - Is to examine the relationship among mindfulness, compassion fatigue, and turnover intention.
Procedures - After you consent, this study would involve you completing the following steps:
· Complete a 35-minute, anonymous, online survey.
Here are the types of questions you will be asked:
I could be experiencing some emotion and not be conscious of it until some time later.
I break or spill things because of carelessness, not paying attention, or thinking of something else.
I notice changes in my thoughts and feelings while I’m at work.
I’m aware of how my body feels during different work tasks.
I have felt trapped by my work.
I have thoughts that I am not succeeding in achieving my life goals.
I plan to stay in my present job for a while. (R)
I am fairly sure I will quit this job in the next year.
Voluntary Nature of the Study - Research should only be done with those who freely volunteer. So everyone involved will respect your decision to join or not.
If you decide to join the study now, you can still change your mind later. You may stop at any time.
Risks and Benefits of Being in the Study - Being in this study could involve some risk of the minor discomforts that can be encountered in daily life such as sharing sensitive information. With the protections in place, this study would pose minimal risk to your wellbeing. If distress arises, support and referral can be obtained via dialing 988 for a national crisis hotline.
Like most research, this study offers no direct benefits to individual volunteers. The aim of this study is benefit society by understanding the factors that affect nurse retention, and reduce nurse turnover. Results of this study will be published in Scholarworks and possibly other research sources that are easily found if you search online for the researcher’s name along with the study topic.
Payment - There will be none.
Security - The researcher is required to protect your privacy. Research files will be kept secure by password-protected devices and platforms.
Protecting your responses - The researcher is authorized to publicly share anonymized group patterns from all volunteers’ responses. The researcher will not use your responses for any purposes outside of this research project. Questionnaire responses will be destroyed after a period of 5 years, as required by the university.
