2021 Needs Assessment - Consumer Survey

The Newark EMA HIV Health Services Planning Council's Research and Evaluation Committee is conducting a survey on the needs of people regarding health issues. The purpose is to determine the need for HIV medical care in person and by “telehealth” and how best we can allocate [Ryan White and other] resources for medical care. Telehealth includes medical care provided by cell phone, video, tablet, computer, but not in person. This will take only a few minutes to complete. Your participation is voluntary, your responses will be kept confidential, and you can decline to answer any of the questions.
