School Climate Survey Permission Form
As part of its strategic plan, Washington Township Public Schools plans to conduct a School Climate Survey for all students in grades 4-12. The survey may be read to younger students, administered in a small group or individually as needed, and allows students to choose picture icons for responses. The data collected will assist the district in identifying areas of strength and areas that need improvement so that we may continue to provide your child with the best educational environment possible. All responses will be anonymous, and the survey will be conducted electronically through Qualtrix by Rowan University's Center for Access, Success, and Equity during the month of March 2020. Please complete the form below no later than Monday, March 16 to indicate if your child has your permission to participate in this survey. Please complete one form for each child you may have in grades 4-12. Thank you for your support and assistance!
