PLEASE READ THIS PAGE CAREFULLY BEFORE BEGINNING YOUR SUBMISSION

Use this questionnaire to submit your personal information and payment information for the TBGO trip to the Spokane Lilac Festival in May 2019. You may also use this survey to let us know that you will be attending the Spokane Lilac Festival Parade as part of TBGO if you are planning to make all your own travel arrangements.

IF YOU SUBMIT THIS FORM, YOU ARE COMMITTING TO MAKE THE TRIP. Your credit card will be charged for deposit and total package cost as described in the full trip description on the TBGO Event Information page at www.thebeatgoesonmb.org/eventinfo.htm. 

Send a SEPARATE FORM for each participant. DO NOT put two people on the same form. So if you and your spouse or roommate or mailman are both planning to travel, please send two separate forms -- one for each person.

Please read carefully - there's a lot of data here. IT IS CRITICAL THAT YOU READ THE INSTRUCTIONS on each field.  Be sure to pay attention which field is your FIRST name and which is your LAST name.

USE PROPER FORMATTING for each field as specified; not only does it make our job easier, but your Credit Card number may be mangled if you fail to follow directions.

While completing the form, you will be asked for the following information:

  • Contact information (name, address, phone, email)
  • Whether you will be attending the Spokane Lilac Festival Parade
  • If you wish to reserve a travel package (or whether you are making all your own travel arrangements)
  • Which motor coach pickup point you will use
  • Name(s) of preferred roommate(s)
  • Whether you are a TBGO Band Associate performing with the band (including Support Staff) or are a family member traveling with the band
  • If you are a Band Associate, which section you are in
  • Whether you want trip insurance
  • Payment information (credit card info or "Will call Tom")
  • Any dietary restrictions (for Old Spaghetti Factory dinner)
  • Any additional information regarding travel arrangements or anything else about the trip that you wish to tell us.

Please make sure you have all the necessary information before beginning the form; otherwise, you'll just have to come back and do it again later on.
