The Virginia Department of Education (VDOE) has been charged with developing procedures to measure the academic and functional progress of students enrolled in private schools for students with disabilities that are licensed by the VDOE through several outcome measures. The purpose of this survey is to solicit input from parents or guardians on how their child is progressing since being enrolled in a private school for students with disabilities licensed by the VDOE. Your participation in this survey, as one measurement tool, will assist VDOE in collecting information necessary to measure the individual progress of a student enrolled in a private school for students with disabilities. It is important to note that your response will be recorded anonymously and cannot be personally linked to you or your child.

Parents or guardians who have more than one child receiving special education services in a private school for students with disabilities should submit one survey for each child receiving services.
