I agree to abide by the rules, regulations and policies of Queensland Athletics, Athletics Australia, the relevant Regional Sports Associations and High Cadence, including Athletics Australia’s Member Protection and Privacy Policies, Anti-Doping Policy and Risk Management Policy (these are available at www.athletics.com.au)

I authorise High Cadence to use and disclose, to related and relevant bodies, any of my personal information that may be necessary to implement the rules, regulations and policies according to Athletics Athletics Constitution and Policies. I agree to have my name and results published in official programs, newsletters and websites.

I am the parent or guardian of the minor named below and I have the legal authority to execute this release. For valuable consideration received, I give High Cadence the irrevocable right to use photographs and video footage of the aforementioned child in all forms, media and manners, without restriction as to changes or alterations, for advertising, trade, promotion, research, training, exhibition, or any other lawful purposes to promote High Cadence. I waive any right to inspect or approve the photographs or electronic matter that may be used in conjunction with them now or in the future, whether that use is known to me or unknown, and I waive any right to royalties or other compensation arising from or related to the use of the photographs. I understand that the name and contact information of the child will be withheld from public disclosure.

I agree to comply with the spirit and principles of High Cadence and the High Cadence Code of Conduct.
