Understanding Our Community Needs

Thank you for taking time to participate in the Adelaide PHN (APHN) National Psychosocial Support (NPS) Measure Survey. This survey is open to health professionals, clients, carers, general community, service providers and NGOs to share their knowledge and experiences to assist in the development of new services. This survey should not take more than 15 minutes to complete.

Please complete the survey before Friday, 26 October 2018.


APHN is currently undertaking a Needs Assessment to determine current and emerging psychosocial support needs in the region. This information will be used to develop and commission new psychosocial supports for the community. The target group for this project are people with severe mental illness, who are not eligible for the NDIS.
Note: This funding excludes clinical care or care coordination/case management.

Psychosocial Supports are non-clinical supports that assist people in their recovery such as:
  • social skills, family connections and friendships;
  • managing daily living needs;
  • financial management and budgeting;
  • finding and maintaining a home;
  • vocational skills and goals, including educational, training goals and volunteering
  • maintaining physical wellbeing, including exercise;
  • managing drug and alcohol addictions, including tobacco; and
  • building broader life skills including confidence and resilience.

Information provided through this survey will be utilised ONLY by APHN for our planning and needs assessment purposes.

All information will be securely stored and used for the purposes for which it is intended and will not be provided to any other third party.
