SEEK B is ASHM's Hepatitis B Case Finding and Clinical Auditing Pilot project. The project goal is to increase the number of people identified for screening, testing, linkage to care, and management for hepatitis B in primary care, thereby reducing the prevalence and mortality rates of hepatitis B in Australia.
This form is to express a primary care clinics interest in being a pilot site involved in SEEK B.
The form will take approximately 7 minutes to complete.
All information you provide will remain confidential and be managed in accordance with the Privacy Act 1988 (Cth) and the Australian Privacy Principals. Aggregate de-identified findings may be used by ASHM for project evaluation and quality improvement, or reported to project funders, in sector conferences and/or through other channels.
