Rising Stars Music Skills absence form for current students

Rising Star students are expected to attend all Music Skills classes. If an outside commitment does arise, an advance notice of absence may be requested. This is essential as we have a duty of care to students and need to know their whereabouts if they are not in scheduled classes.

Please use this form to explain the reason for your requested absence on a required date. We require one form per student. This information will be used to record the date and reason of any students absence.
