Healthy Together Expression of Interest - New Facilitators

Families Canada, through a new philanthropic partnership with Sun Life Canada and Dialogue, invites Healthy Together program facilitators to express their interest in participating in a 1-year initiative to gain access to no-cost virtual healthcare for you, your dependent family members, and participants of your Healthy Together program sessions.

Families Canada member organizations participating in the program will receive:
  • Training for two staff to become Healthy Together certified facilitators.
  • A stipend to purchase groceries and materials to deliver the first Healthy Together program in your centre.
  • Access to a network of facilitators delivering the program through a Community of Practice.
The program will include:
  • No cost, unlimited access until November 30, 2026 to Dialogue’s virtual healthcare services, available through their app or website.
  • The ability to meet with healthcare professionals 24/7 through on-demand video or chat appointments, and receive personalized health plans, prescriptions (drug coverage not included), specialist referrals, and lab tests – as applicable.
  • Informational materials containing the details of this offer to share with the participants of your Healthy Together program sessions.

Capacity and Commitment

In expressing interest in this training and access to Dialogue Primary Care, I understand and agree that:
  • The information I share in this form will be shared with Sun Life.
  • I will be expected to attend one 30-minute Dialogue training webinar. These sessions are pre-recorded with live Q&A.
  • I will be encouraged to register for an account on the Dialogue app and create a personal profile.
  • I will be expected to exclusively promote this opportunity to the families participating in the Healthy Together program sessions which I am facilitating.
  • Two staff from my organization will complete the Healthy Together online facilitator training (Families Canada Training Portal).
  • We will deliver the Healthy Together program in our centre at least once.
  • We will share results from the program delivery with Families Canada.
