PrEP Clinic - Community Feedback Survey

Cabarrus Health Alliance PrEP Clinic

In an effort to expand preventative services around HIV, Cabarrus Health Alliance (CHA) will start offering PrEP in August 2016, with appointments available on Wednesdays from 8:00am to 4:30pm. We are seeking feedback from the community about the addition of these services to our operations.  Comments and suggestions regarding the PrEP clinic are welcome through our online survey and will be taken into account as the program develops. Please provide any and all feedback.
1.Do you have any general comments, suggestions, or other feedback about the new PrEP clinic at Cabarrus Health Alliance?

2.Will the availability of a PrEP clinic at Cabarrus Health Alliance directly affect you, someone in your household, or someone in the community that you know?

3.If you answered yes to Question 2, how?
4.Have you heard of PrEP before taking this survey?

5.If you answered yes to Question 4, then have you, someone in your household, or someone in the community you know used PrEP before?
6.In addition to Wednesdays 8:00am – 4:30pm, what would be convenient hours of operation for the PrEP clinic if you were to use these services?

7.Do you believe the PrEP clinic will have a positive impact on the community? Why or why not?
8.How likely are you to use or recommend these new services?
