Select one answer for each of the following statements.

This is a simple 25-question assessment designed to measure your organization's culture of philanthropy. Each answer is assigned a numerical score. At the end of the survey, the survey will calculate a total score for you. This score is used to determine the health of your organization's culture of philanthropy and provide insights on how it can be improved.

If you are unable to answer a question, you may provide a comment in the "Other" field. This will impact your final score.

At the end of the survey, you will be invited to provide your contact information so we can email your philanthropic culture assessment to you to share with others in your organization.
