The Healthy Start Coalition of Flagler and Volusia Counties is conducting a community needs assessment. As a community partner, leader, and stakeholder in this area, your perspective and opinions on the health and needs of pregnant women and their families are important. Your input will help us better understand local unmet needs and service/resource gaps. This information will guide decisions on how to improve services and better serve the needs of pregnant women and families in your community.
This survey should take no more than 10 minutes to complete. This is an anonymous survey and your responses will be kept confidential. None of the information you provide will be used to identify you. To be eligible to take the survey, you must be 18 years of age or older, and provide services or resources in Flagler or Volusia counties.
