Group Entry Form

Thank you for applying to be a contestant for HOPE You Got Talent.

This Group Entry Form is for a group entry and must be completed in its entirety.

HOPE of East Central Illinois and the Eastern Illinois University Police Department reserve the right, exercisable at any time in its sole discretion, to disqualify you or any member of your act from the audition process and competition should such individual(s) supply untruthful, inaccurate or misleading personal details or information, or for any other reason whatsoever.

After submitting the Group Entry Form, each member of the group must complete the individual submission form for group entry.

For Group Acts, after the Group Entry Form and Individual Entry Forms are filled out please submit your Audition Video Link via email to info@hope-eci.org.

All entries must be received by September 1, 2019. Acts are encouraged to submit prior to August 1, 2019 to ensure equal time for public vote.
