This online form may be used by PQC students or employees, or others (not affiliated with the College) who have been involved in a possible Title IX incident involving a PQC student or employee.
1.If you wish to make an anonymous complaint, leave your name blank. In order for the College to fully investigate this complaint, your contact information will be needed. During a Title IX Investigation, the College cannot withhold your name from the alleged perpetrator. The details of a Title IX investigation are not public information and will be maintained in confidential files within the Title IX Office.
