Before you fill out the electronic form below, we suggest you download a sample nomination form and fill it out prior to submitting the electronic version.

The form will guide you to the appropriate questions for the category you wish to nominate your candidate. Once completed off-line, we suggest submitting it electronically. Our system does NOT allow you to save your submission if it’s only partially complete. You’ll see in the form that we ask 2 questions for each category with responses that are no longer than 350 words per question.
