Applicants intending to apply for a new or re-competing Formula program are REQUIRED to complete this Notice of Intent to Apply form by 3:00 p.m. CT on February 20, 2026.

Failure to submit a Notice of Intent to Apply will render a new or re-compete applicant ineligible to apply.

Continuation grantees are not required, but encouraged, to submit a Notice of Intent.
