VCS Technician Survey

Stokes Pharmacy is excited to announce that we have invested in a new enclosed tablet machine specifically for chemo that is much safer for our staff and your staff. It is also more accurate and cost effective versus capsules. We are in the process of choosing which chemotherapy tablets to offer. In an effort to limit the number of tablet strengths we make, we are asking for your input. Keep in mind, because our chemotherapy tablet list will be limited, we will continue to compound capsules for customized strengths. Thank you in advance for participating; your opinion is valuable to us.

Question Title

* 1. Does your office/hospital stock compounded medication?

Question Title

* 2. What compounding pharmacy does your hospital use for office use and/or direct to client prescriptions. Why?

Question Title

* 3. In your practice, who makes the decision on which pharmacy to order compounded medications from?

Question Title

* 4. When prescribing 3.7 mg of compounded drug x, did you know that you are actually getting anywhere between 3.3 mg and 4.1 mg (and this variance is even greater at some pharmacies)?

Question Title

* 5. Stokes plans on formulating chlorambucil, cyclophosphamide, lomustine, melphalan and piroxicam tablets in limited strengths. Is there a drug you would like to see on this list that is not?

Question Title

* 6. If your oncologist prescribes chlorambucil tablets and had the choice of limited strengths, would the following: 1 mg, 1.5 mg, 1.75 mg, (2 mg is commercially available and will not be compounded), 2.25 mg, 3 mg, 3.5 mg, 4 mg, 4.5 mg, 5 mg encompass 80% or more of your patients?

Question Title

* 7. As you may be aware from the JAVMA article regarding potency of compounded chemo, manufactured as well as compounded medication can be within +/- 10% of the labeled potency. Question Title

* 8. If your hospital could legally stock, dispense and administer compounded chemotherapy for office use, do you feel this would improve the care you provide to your patients?

Question Title

* 9. Did you know there is a Mechlorethamine HCL (Mustargen®) preparation available for purchase that does not require filtration?
