Innovation Grant: Funding Request

If your grant has been approved, this online form must be used to request funds for your approved innovation grant project. It should be completed by the primary grant recipient or a TLED staff member. Once this form is submitted, it will be verified and forwarded for processing. If there are any discrepancies, you will be asked to review/revise your request.

Category: Equipment & Supplies 
(Item Name, Description and # of items, Suggested Vendor & Total Cost)
Category: Contractual 
(Service(s) provided. Description, Contractor/Vendor Contact name & phone, Amount Due)
