Questions to assess attitudes toward the vaccine:

Please Complete This Survey Only Once. Thank you for your participation

The purpose of the survey is to gauge the Acoma community attitudes, perceptions, and knowledge related to the COVID-19 vaccine for children ages 5 - 12. The Pfizer Corp recently asked the FDA for Emergency Use Authorization (EUA) for young children. As the Acoma Dept of Education and POA HHSD prepare for vaccination clinics, your participation in this survey helps us with our planning efforts. The vaccine clinics have not been scheduled until we receive final word from the New Mexico Dept of Health that the vaccine is available.

The proposed vaccine for children is a two-dose regimen; the doses are the equivalent of a third of a shot given to an adult and is administered three weeks apart.

Early studies reveal that two weeks after their second dose, children develop an antibody response equivalent to teenagers and young adults who have been vaccinated with the full dose.
