* 1. Informed Consent
Greetings!

Dr. Nicholas Carter, a Professor at National Louis University, is inviting you to participate in this study: “Implications for the Psychological Sense of Community and Structural Power Theory for Employee Empowerment Research.” The purpose of this study is to determine if organizational factors and the psychological sense of community contributes to feelings of empowerment among workers in mental health spaces.

This form outlines the purpose of the study and provides a description of your involvement and rights as a participant. By selecting YES , you are providing consent to participate in a research project conducted by Dr. Nicholas Carter.

Please understand that the purpose of the study is to explore the contributions of specific organizational factors (access to information, access to resources, and access to support) to feelings of workplace empowerment among mental health professionals. Additionally, this study will examine how the psychological sense of community also contributes to feelings of empowerment in these spaces.

Participation in the study will include the completion of a demographic survey as well as 3 other surveys via Survey Monkey. All surveys are compiled together and presented as a single document. There are total of 54 items and should take approximately 25 – 30 minutes to complete. Please make sure to read each statement carefully before answering. Please respond to each statement with your level of agreement ranging from Strongly Agree to Strongly Disagree. For items that include the terms “organization” or “company,” please answer with your primary mental health work setting in mind.

Your participation is voluntary. You may refuse to participate or opt out of the study at any point with consequence.

There are no anticipated risks or benefits, no greater than that encountered in daily life. However, there may be potential for mental health leaders to better understand the implications of empowerment and a sense of community within their organization. This may lead to leaders taking steps to improve and foster the sense of empowerment and community among their employees - leading to a more harmonious and efficient work culture.

Upon request you may receive summary results from this study and copies of any publications that may occur. Please email the researcher: Dr. Nicholas Carter at Dr.N.Carter13@gmail.com to request results from this study. Also, you may email the researcher if there are any questions or concerns about the study.

If you have any concerns or questions before or during participation that has not been addressed by the researchers, you may contact the cochairs of NLU’s Institutional Research Board: Dr. Shaunti Knauth; email: Shaunti.Knauth@nl.edu; phone: (312) 261-3526; or Dr. Carla Sparks; email: Csparks3@nl.edu; phone: (813) 928-6889.
