Champlain Hospice Palliative Care Program

* This free course is only open to healthcare staff working within the region of Champlain, which include Ottawa, West to Barry's Bay, and East to Hawkesbury. For a map of the Champlain region, please see: https://www.champlainhealthline.ca/aboutourregion.aspx?region=Ottawa

*LEAP Online is for health care professionals (e.g. physician, RN, RPN, pharmacist, social worker, etc.) who provide care for patients with progressive life-limiting illnesses.

* We are not accepting Personal Support Workers, volunteers, or unpaid family caregivers into this course. We are organizing different palliative care courses specifically for these groups.

* For a limited time, apply to join a LEAP Online course at a 100% discount. LEAP Online usually costs $350 for physicians and $250 for non-physicians (plus taxes).

* There are a limited number of spaces in the free LEAP Online courses, so we unfortunately cannot accept all applicants. Priority will be given to applicants who demonstrate that they are willing to commit to completing all of the online modules (approximately 8 hours of work) and the online webinars (6 hours).

* Registrants must complete all of the 16 online modules before the first webinar takes places in order to receive a certificate of attendance.

7.I am able to complete all of the 16 online modules (approximately 8 hours of work) prior to the webinar. This is a requirement in order to be registered in the course and to receive a certificate of completion. If you do not complete the online modules prior to the first webinar, Pallium will not let you into the webinar and you will not receive a certificate.(Required.)
8.How committed are you to completing all of the online modules (approximately 8 hours of work) prior to the first webinar and attending the two webinars (6 hours)? 
If you are accepted into the course, you will receive an email before the start of the training to confirm your spot in the session. The email will let you know which session date and time you were accepted for. If you are accepted into the course, please provide at least 24 hours' notice if you need to cancel. Failure to notify us within this timeframe will result in your future applications being waitlisted for the following year.

Failure to attend sessions that you have registered for prevents others from the opportunity to attend. It is important that if you cannot make a session that you have registered for that you take the appropriate steps, so that someone on the waiting list could fill your seat.
